Please fill out the release forms below. These forms are mandatory and must be submitted before participating in the summer camp.

Parents: If you have multiple children participating in the camp, then please feel free to list all of their names in the first name columns. You do not have to fill out separate forms for each child.

DESCRIPTION OF ACTIVITY: Participation in The Dr. Shirley Linzy Young Artists Orchestra of Las Vegas rehearsals, programs and activities as a member or guest In consideration for the above-named Participant being allowed to participate in the above-described activity (“Activity”) at The Dr. Shirley Linzy Young Artists Orchestra of Las Vegas (the “YAO”), the undersigned agrees as follows:

ON BEHALF OF OURSELVES, HEIRS, ESTATES, AND FAMILY MEMBERS, I HEREBY FULLY RELEASE AND DISCHARGE LVYAO AND ITS EMPLOYEES, OFFICERS, AGENTS, BOARD MEMBERS, CONTRACTORS, AND ASSIGNS (THE “RELEASED PARTIES”) FROM ANY
LIABILITY, INJURY, ILLNESS, LOSS, DAMAGE OR CLAIM TO ME OR MY PROPERTY ARISING FROM THE ACTIVITY, WHETHER CAUSED BY NEGLIGENCE OF THE RELEASED PARTIES OR OTHERWISE.

I acknowledge that this Release of Liability includes a full release of the Released Parties for any damage or loss to property of the Participant or Participant’s Parents or Guardians.

FOR PARENTS/GUARDIANS OF MINOR PARTICIPANTS
This is to certify that I, as parent/guardian with legal responsibility for the minor Participant named above, do consent and agree on his/her behalf to all of the foregoing  conditions to participation and to the release as provided above of all the Released Parties, and, for myself, my child and our heirs, assigns, and next of kin, I release and agree to indemnify and hold harmless the Released Parties from any and all liabilities incident to my minor child's involvement or participation in the Activity, including, without limitation, any claims that minor child may make against the Released Parties and any claims that I may have as a result of any injury to my minor child or property loss, even if arising from the negligence of the Released Parties, to the fullest extent permitted by law.
